Hope in Action Practitioners are mentors, coaches or therapists who will walk with you on your journey to positive change, with support from family therapists and other professionals where it will be beneficial to you.
The programme is delivered through a 5D model, with each D referring to an overarching aim for that section of the programme. Our key approach is systemic narrative practice – put simply, this means that we want to hear your story and work with you to create and live the story that you want to be part of.
The first phase of the programme is Hope and will cover 12 sessions:
Discovery (6 sessions)
· Getting to know each other and the programme
· Telling your story
· Identifying a starting point so you can see your progress during the programme
Dream (3 sessions)
· Understanding your motivation for change
· Identifying what matters to you
· Setting goals to work towards
Design (3 sessions)
· Exploring and working with your significant others (if you wish to)
· Looking at your social support network and resources in the community
· Co-creating a positive change plan
If you want to continue your journey with us, you can progress to the second phase of the programme - in Action. This phase provides 24 sessions:
Delivery (24 sessions)
· Supporting you with your personalised positive change plan
· May include working with partner organisations where this would be helpful to you (but we will walk alongside you with this)
· Will include the opportunity to engage in group sessions/courses/taster workshops/activities and outings etc.
· Will include advocating for you if this would be helpful
Development (1 session)
· Exploring what has worked well, and not so well, for you and other programme participants to evaluate and refine the programme for the future.
How we work together
When we have received a completed selection form for you, we will talk to you about your availability for support sessions and your preferred location for these sessions, as well as gathering some additional information.
We will then introduce you to a practitioner who will work with you throughout the programme. Before you first meet, we will share a photo of the worker and a short piece of information about them with you.
You will organise your sessions between you and the practitioner will agree contact methods and working arrangements with you.
Your practitioner will also talk to you about the data we collect and how we will use it, and you will be asked to sign an RSVP form consenting to this.

We will share our Comments and Complaints Policy with you and are happy to share any other policies you would like to see, please ask your practitioner or contact us on the email above.
We have had initial contact with the other drug and alcohol services in Birmingham and are keen to work in partnership with them where it will be of benefit to our participants. We can either refer our participants into their services, where additional treatment or help is required, or we can explore the possibility of accepting their service users onto our programme.
We are aiming to support people who haven’t accessed drug or alcohol services before. We are not a medical/clinical service, and we know that it can be very dangerous for people who are dependent on alcohol or opioids to stop using these substances without medical intervention. There are other services who can offer this support.
This service is funded by DATRIG monies and there is no cost to the participant or referring organisation.
We do not have premises of our own. We intend to meet people where they feel most comfortable. This may include their home, a community centre, a café etc.
Our practitioners are a small team of passionate, local professionals consisting of experienced mentors, coaches and therapists.
